On June 18, 2021, Maximus, as state contractor to Ohio Medicaid providers, sent notice that it “experienced a data breach in which the personal information of Medicaid healthcare providers” was compromised. It was determined Maximus’s “server was impermissibly accessed starting on May 17, 2021.” The breach was discovered on May 19, 2021.
The Maximus data breach potentially exposed provider names, social security numbers, addresses, and other information, and affects 334,690 individuals.
