Great Valley Cardiology disclosed the data breach involves sensitive confidential information, including:

  • Names;
  • Addresses;
  • Credit/Debit Card Numbers;
  • Dates of Birth;
  • Social Security numbers;
  • Driver’s License and Passport numbers;
  • Bank Account information; and
  • Health Insurance/Medical Information.[3]
